If you have an HMO plan coverage, you must select a primary care physician (PCP). This selection must be done prior to your visit to a physician in order to assure you will be covered for the visit by your insurance, and in order to assure that any necessary referrals can be processed for your healthcare benefit.
In order for us to submit your claim for insurance payment, you must present a valid insurance card at the time of your visit. If your insurance card has a primary care physician listed, please be sure that your provider's name is on the card.
Referrals To a Specialist
If your insurance requires a referral in order to visit a healthcare specialist, this referral will require an office visit with your Primary Care Physician. If you do not have a referral from your primary care physician prior to seeing the specialist, your insurance may not pay for the visit. Please be sure your physician's name is listed as the PCP with your insurance carrier. If not, this oversight can significantly delay your referral. Please allow plenty of time to obtain your referral to a medical specialist. We request that you allow three business days to process your referral.
