We realize you have a choice of healthcare providers and appreciate your trust in us. We value your time and we want your visit to address your concerns effectively and in a thorough manner. The following tips should ensure that your visit goes as smoothly as possible:
1. Please arrive 10-15 minutes earlier than your appointment time in order to complete necessary paperwork for registration and insurance. You may also print the forms from our website and complete them prior to your arrival.
2. Annual physicals are intended to address preventative care issues, such as immunizations and cancer screening. In order for us to have enough time to provide preventative care, we prefer not to address acute medical problems during the annual physical. Should you have an acute care issue, please be sure to tell our staff at the time of scheduling so the correct type of appointment can be made.
3. In order for us to thorougly evaluate your healthcare needs, it is best to plan to address one new health issue, or two total problems, during a visit. In this manner, we can focus more effectively on your greatest concerns.
4. The best doctor/patient relationship is an open and honest one, so please don't hesitate to tell us exactly what you are thinking, or if you are worried about something specific. Open communication between the physician and patient will lead to the best healthcare results.
5. Please bring your medications with you to your visit so we can ensure that our records are correct. You may also bring an updated list of the medication names, dosages, and number of doses taken per day. We are not always notified about changes in your medications done by other physicians, so please be sure to tell us about medications that may be prescribed by other doctors that you see.
6. Please tell us if you are taking any over-the-counter or herbal medications, because they may interact with medications we prescribe.
7. Your family's health may affect your personal health, so ask your family members about their medical history. Bring a copy of your family's history so we can include this in your chart.
8. We want you to get the most out of your visit. If you prioritize your concerns to us at the beginning of the office visit, we can address directly the problems you are most concerned about.
9. We ask that you turn off your cell phone, so that we can verify your information and communicate with you without distraction or interruption.
10. Please bring your insurance card to each visit, in order for us to keep accurate records that will facilitate insurance coverage of your visit.
